Here at Preveon, we have dedicated Patient Care Specialists ready to take your phone call and begin the enrollment process. You can reach us directly by calling (909) 693-3376 during our business hours. Once you are connected with a Patient Care Specialist, we will take down some basic demographic and medical information and then initiate the enrollment process.
2) Talk To Your Provider
You can have your Provider fill out a Preveon Patient Referral Form, which will provide us with the necessary information to begin the enrollment process.
Preveon - Blood Pressure Monitor and Small Volume Nebulizer
Preveon offers high quality blood pressure monitors and small volume nebulizers to our patients through select insurance plans. Please contact us for more information.
With our mail-order pharmacy, patients with select insurance plans not only receive their supplies from the comfort of their homes but also have the opportunity to talk to once of our clinical pharmacists and registered dietitians regarding their health and medications.
